YOUR FIRST VISIT
Your first visit will last 15 minutes. Your Therapist will guide you in providing us with information using:
· An Extensive History Form
· A Four Day Bladder Diary
The Bladder Diary is particularly important. It must be done for 4 consecutive days. Thereafter, an appointment should be made to begin your treatment plan. You will need to sign a Consent for Evaluation and Treatment Form on the day of your first treatment.
